Social anxiety disorder (SAD) — sometimes called social phobia — is more than shyness. It involves an intense, persistent fear of being watched, judged, or humiliated in social or performance situations. This fear is disproportionate to the actual threat and often leads people to avoid situations entirely — skipping meetings, declining invitations, avoiding phone calls, or staying silent when they have something to say.
According to the National Institute of Mental Health, social anxiety disorder affects approximately 7% of U.S. adults. It is one of the most common anxiety disorders, and it typically begins in adolescence. Despite its prevalence, many people with social anxiety go years without treatment — partly because the disorder itself makes seeking help feel threatening.
The evidence base for treating social anxiety is strong. SSRIs — particularly sertraline and paroxetine — are recommended as first-line pharmacological treatment. CBT with exposure therapy is the gold-standard psychotherapy approach. Your first appointment includes a thorough assessment of your social anxiety symptoms, their impact on your work, relationships, and daily functioning, your medical history, any past treatments, and co-occurring conditions like depression, other anxiety disorders, or substance use.
SSRIs as First-Line Treatment
SSRIs are the most evidence-supported medications for social anxiety disorder. Sertraline (Zoloft) and paroxetine (Paxil) have the strongest research base for SAD specifically, though escitalopram and other SSRIs are also effective. SSRIs reduce the physiological fear response in social situations over 4–8 weeks and have no abuse potential.
SNRIs as an Alternative
Venlafaxine (an SNRI) is also well-supported for social anxiety disorder, particularly when SSRIs haven't provided adequate relief or when depression co-occurs. SNRIs target both serotonin and norepinephrine, which can be beneficial for patients with prominent physical symptoms like blushing, sweating, or trembling.
CBT With Exposure Therapy
Cognitive behavioral therapy for social anxiety involves identifying negative predictions about social situations ('Everyone will notice I'm nervous'), testing those predictions through gradual exposure, and building new patterns of thinking and behaving. We coordinate with therapists who specialize in exposure-based CBT for social anxiety.
Adjunct Medications When Needed
For specific performance situations (like public speaking), some patients benefit from additional support. Hydroxyzine can help with acute situational anxiety without addiction risk. Buspirone may be added for generalized anxiety symptoms that accompany social anxiety. Your psychiatrist will discuss whether adjunct medications are appropriate for your situation.
