Generalized anxiety disorder (GAD) is characterized by persistent, excessive worry about a variety of topics — work, health, finances, family, everyday matters — that is difficult to control. Unlike the focused fear of panic disorder or social anxiety, GAD involves a broad, free-floating anxiety that shifts from subject to subject and rarely lets up. People with GAD often describe themselves as "always worried" or "unable to turn off their brain."
The National Institute of Mental Health reports that GAD affects approximately 3.1% of U.S. adults in a given year, with women being affected about twice as often as men. GAD often develops gradually — many people have lived with chronic worry for so long that they assume it's just their personality. In reality, GAD is a medical condition with well-established, effective treatments.
GAD is one of the most common conditions our board-certified psychiatrists treat. First-line treatments include SSRIs, SNRIs, buspirone, and cognitive behavioral therapy (CBT). At Empathy Health Clinic, we begin every treatment relationship with a thorough evaluation — understanding your symptoms, history, and goals before recommending any medication or therapy approach.
Chronic Worry vs. Normal Worry
Everyone worries sometimes. Here's how GAD differs from everyday concern:
Normal Worry
- • Proportional to the situation
- • Temporary — resolves when the issue does
- • Doesn't significantly interfere with functioning
- • Can be set aside when needed
GAD Worry
- • Excessive and out of proportion
- • Persistent — most days for 6+ months
- • Interferes with work, sleep, and relationships
- • Difficult or impossible to control
If your worry matches the right column, a psychiatric evaluation can determine whether GAD — or another condition — is the cause.

How We Treat GAD
A comprehensive, long-term approach to managing chronic worry
Comprehensive Psychiatric Evaluation
GAD shares symptoms with several other conditions — including thyroid disorders, other anxiety disorders, depression, and ADHD. Your first visit includes a thorough assessment to confirm the diagnosis, identify co-occurring conditions, and understand your symptom history before any treatment recommendations.
SSRIs as First-Line Treatment
SSRIs — including escitalopram, sertraline, and paroxetine — are among the most commonly prescribed first-line medications for GAD. They reduce the intensity of chronic worry and physical anxiety symptoms over 4–8 weeks. SSRIs have no abuse potential, are safe for long-term use, and also address co-occurring depression when present.
SNRIs for GAD
Serotonin-norepinephrine reuptake inhibitors like venlafaxine and duloxetine have strong evidence for GAD treatment. SNRIs may be particularly helpful when GAD includes prominent physical symptoms (muscle tension, fatigue, headaches) or co-occurs with chronic pain. Like SSRIs, they carry no addiction risk.
Buspirone
Buspirone is specifically FDA-approved for generalized anxiety disorder. It works through a different mechanism than SSRIs or SNRIs, has no abuse potential, and does not cause sedation or dependence. Buspirone can be used alone or in combination with an SSRI/SNRI when additional anxiety relief is needed.
Cognitive Behavioral Therapy (CBT)
CBT for GAD focuses on identifying the patterns that maintain chronic worry — overestimating threat, underestimating coping ability, and intolerance of uncertainty. Through structured exercises, CBT helps you respond to worry differently and develop more adaptive thinking patterns. We coordinate with therapists who specialize in CBT for anxiety.
Ongoing Medication Monitoring
GAD is typically a chronic condition that benefits from sustained treatment. We provide regular follow-up appointments to track medication effectiveness, manage side effects, and adjust your plan over time. Our approach includes both in-person and telehealth options for convenient, continuous care.

The Physical Side of GAD
GAD is not just excessive worry — it manifests physically throughout the body. Many patients initially seek medical care for physical symptoms before recognizing anxiety as the root cause.
Muscle Tension
Chronic tightness in neck, shoulders, jaw, and back
Sleep Disruption
Difficulty falling asleep, staying asleep, or waking unrested
Fatigue
Persistent tiredness despite adequate rest
Headaches
Tension headaches from sustained muscle contraction
GI Problems
Stomach upset, nausea, IBS-like symptoms
Concentration Issues
Mind going blank, difficulty focusing on tasks
Effective GAD treatment addresses both the mental and physical symptoms. SSRIs, SNRIs, and CBT all improve physical anxiety symptoms alongside worry.
