SAMHSA guidelines (TIP 63) emphasize that MOUD should include regular follow-up, assessment of treatment response, toxicology monitoring when appropriate, and ongoing clinical decision-making. Our monitoring protocol follows these evidence-based recommendations while adapting to each patient's individual needs and recovery stage.

Monitoring is not about policing — it is about accountability and safety for both patients and providers. Structured follow-ups create regular touchpoints where medication can be optimized, co-occurring conditions can be assessed, and relapse prevention plans can be updated. Patients who receive consistent monitoring have better treatment retention and recovery outcomes.

What Monitoring Looks Like at Each Stage

Early Treatment (Weeks 1–4)

Weekly or biweekly appointments. Focus: medication induction, dose stabilization, withdrawal management, side effect assessment, co-occurring condition evaluation, and establishing the treatment alliance.

Stabilization (Months 2–6)

Biweekly to monthly appointments. Focus: medication optimization, coping skill development, trigger identification, relapse prevention planning, and coordination with therapy providers.

Maintenance (6+ Months)

Monthly appointments (or as clinically appropriate). Focus: treatment plan review, long-term medication management, recovery support, periodic safety assessments, and discussion of treatment goals including potential taper when appropriate.

Regular Follow-Up Appointments

Scheduled visits at a frequency appropriate to your recovery stage. Early treatment typically involves weekly contact; stable maintenance may move to monthly. Each visit assesses medication effectiveness, side effects, cravings, mood, and overall functioning.

Medication Safety Monitoring

Assessment of medication effectiveness, dose adequacy, side effects, and potential drug interactions — especially important for patients taking psychiatric medications alongside MAT. Includes monitoring for sedation, constipation, and other buprenorphine-related effects.

Toxicology Screening (When Appropriate)

Urine drug screens may be used as a clinical tool to verify treatment adherence, identify unreported substance use, and guide treatment decisions. Screening is conducted as a supportive measure with transparent communication about results and their implications.

Safety Considerations in MAT

Buprenorphine (Suboxone) has a well-established safety profile when prescribed and monitored appropriately. However, like all medications, it requires clinical oversight. Key safety considerations include:

Sedation & Respiratory Depression RiskWhile buprenorphine has a 'ceiling effect' that limits respiratory depression compared to full opioid agonists, combining it with benzodiazepines, alcohol, or other sedatives increases risk. We screen for and address these interactions.
Precipitated WithdrawalStarting buprenorphine too soon after last opioid use can cause precipitated withdrawal. Our induction protocol is designed to minimize this risk through careful timing and patient education.
